Again, Klein was paralyzed at the time, and told to use it at the first opportunity.
Kirito and Asuna are kept alive because of kayaba, not any abnormal means.
It’s pretty clear to me that the strength of will here is the Incarnate system. The connection between the user and the system with the nervgear is greater than the amusphere, but far weaker than the STL so it makes sense that incarnating would be far more difficult in SAO. The pain kirito experiences is also very similar to the pain described when players incarnate too much in the Dark Territory war.
